Both the Etude and the Maestro move the weight of the clarinet to the proximal phalanx of your thumb, while the distal phalanx still remains in contact with the base part of the thumb rest. This makes that you can't move your thumb as freely as you would be able to with a stock thumb rest. Since you're having issues with fast trills (I suppose right hand trills), I think that when playing with a stock thumb rest, you may not only be using your fingers to trill, but that there may be also some movement in your wrist, which wouldn't work that well with an Etude (or a Maestro) thumb rest.
