My 1939 clarinet case interior has become dirty and the crushed velvet is now matted in places. I want to clean it without replacing any of the interior of this vintage case - any suggestions?

I’ve found it’s better to replace old cases. Many (most?) old cases use adhesives that cause keys to tarnish much faster than they normally would. Newer cases like ProTec and Bam also protect the instrument better and have options like straps and large pockets for music that will make your life easier.
For your current case I would just vacuum it out and store it in a safe place in case you ever want to sell the instrument. Stains can be cleaned out to an extent with 99% IPA with a lint free cloth as well.

I don't think vintage cases are worth anything if you refurbish them. Such as for auctions or something. Just get a new one. They are not expensive at all.

With collector's pieces that might be different: I'm cherishing the old etuis, they are little pieces of art for themselves, and sometimes have them covered in real leather by a craftsman and do the interior lining by myself.
Two examples:Arthur Uebel 1936, Carl Kruspe ca. 1925.

you could also try a fabric cleaner like "Duragloss 451" (available on Amazon)- spray it on, brush it gently with a scrub brush, spray with water, vacuum off, and repeat as needed. Will never restore a matted interior to as-new condition but can make a significant improvement.

If you live somewhere with an assured supply of sunshine I've found that cleaning with a vacuum cleaner, a spray with a good antibacterial spray like Glen 20 and several days exposure to harsh sunlight will pretty well kill anything and get rid of case odour.
