 concerto programming

Driving home this evening I listened to the NY Phil perform Nielsen's Clarinet Concerto with soloist Anthony McGill. A question came to mind.

Who decides to play a concerto with one of the orchestra members as soloist? Is it just whoever usually chooses the programming (the conductor?), and they come up to the member one day and say, oh, we're doing the Nielsen next month. Or does the member go up to the (conductor?) and say, hey, I wanna play the Nielsen Concerto. How is the collaboration decided on?
