Use the long F fingering and tremolo to A (no pinky).
You'll probably have better success if you start with the A and tremolo down to the F.
Keep your throat open for voicing.

It's an F-A tremolo as the interval is more than a whole tone.
Who could possibly write an altissimo F-A tremolo anyway? The only thing I can suggest is find by experimenting a suitable fingering to get the A in the lower altissimo (instead of the regular A fingering) so you can then put fingers down for the F.

I'm very interested in knowing what composer would write such a thing, and what piece it's in.

The piece is by Scott McAllister. It is at the very end of the final movement (7th) of his "epic concerto".

Have you tried e-mailing Kimberly Cole Luevano at University of North Texas? She performed it at ICA ClarinetFest 2012 in Lincoln, so she might have suggestions.

Fake it, play around with different side keys and or LH G# -A keys in combination from the F and see if one of them comes close to an A. Any combination of side keys may give you something you can get away with. I wish composers would ask a player if something is possible before writting it.
