Over the last few years my arthritic thumbs have been causing problems when playing the clarinet. I've used the Ridenour thumb saddle and one of the Ton Kooiman thumb rests with some success, but while they make it possible to play with an arthritic thumb they don't address the fundamental problem.
After some research I have undergone a series of PRP injections into my left thumb joint, where the thumb joins the hand. So far I've had 2 of the complete series of 3, with 1 to go next week. The results so far have been extremely encouraging, my thumb has far more mobility and far less pain than for years. There is still some pain, but I'm advised by my medical practitioner that this will reduce over the next few weeks. The process itself is uncomfortable rather than painful.

I'll report later on the longer term effectiveness. If it works as well as initial indications suggest I'll get the other thumb done as well.

I too, have problems in my right thumb, mainly when trying to hold the clarinet, and I do use a rideur thumb saddle. I have found out that I am double jointed, but so far, no luck fighting the pain,

You may find the Ton Kooiman thumb rest to be more helpful than the Ridenour saddle. It transfers the weight to the base of the thumb and tends to spread the load over the whole hand rather than just the thumb.
