Most instrument repair places that do serious work on woodwinds should have a couple of adjustable thumbrests. The simplest kind only adjusts up and down; I'm not sure who provides it. That's the kind I use, and I found that even a slight change in height made a big difference. If you have problems with tendonitis, you might want to check out the (unfortunately significantly more expensive) Kooiman thumbrest. This gets placed at an exact height on your instrument, and you adjust it to put less of the weight on stressed joints/tendons; it also makes it much more difficult to grip the clarinet too hard. Unfortunately, that runs around $200 Canadian. In any case, your best bet is to talk to a trusted instrument repair shop, and they'll help advise you. HOpe this helps!
