Most places are asking for incoming grad students to take the test. If you are not familiar with it, most college campuses have computerized practice exams. I would highly recommend the practice exams so that you can get a feel for the format of the exam. I think the actual exam takes 2-3 hours. Knowing what to expect can help you for the mental endurance of the GRE.

It depends on the school. 16 years ago when I was applying to grad schools, I applied for 5 which did not require it and one that did.

I'm a clarinet performance major and am currently grad school shopping :D None of the schools I have looked at require their performance majors to take the GRE.

When I was grad school shopping, it really depended upon the school as far as taking the GRE. Most performance programs do not require the GRE. Most academic related programs, theory, history, etc. require the GRE. Some schools will require the GRE if your undergrad GPA would put you on probation. Some schools flat out require it. I know that CCM is one of the few conservatories if not the only that REQUIRES the GRE. At least when I was looking there that was the case. As stated above, it really just depends on the school. Most schools will list all of this info on the graduate admissions page.

To add more confusion, some universities require the GRE general test, but Music Departments may not require the Subject test.

I had to submit scores for 2 of the 3 schools where I applied for admission to the MM program. The third recommended it for scholarship consideration. Take as many practice tests as you can, it's useful to get a high score.

Since no one else has mentioned it, check to see if your schools accept the MAT (Miller Analogies Test) as an alternative to the GRE. It's generally more suited to creative/analytical types.
When I was applying to various MFA programs, every single one accepted my MAT scores, and many stated that they actually preferred the MAT to the GRE as it shows whether you can actually THINK. I'd wager the good music schools are of a similar mindset.
