 Format for music-specific CV/Resume

Hey guys

I'm looking for advice on my CV/Resume to be used for post-graduate applications.
What's the best way to set it out, and what should be included and what should be excluded?
Do I need to include any education prior to my undergraduate degree? And should I name ALL music awards, regardless of how small they seem e.g Eisteddfod results?
