Please, if you choose to answer this, do not call into question the rationale for the piercing. We already have a very long thread on general piercing; this person is asking about a specific type. Please answer only if you have direct experience with this type of piercing.
clarinetbandgeekgoddess, the first thing I would do is ask my doctor, just as I would do if I were having any operation in my mouth. Just give him/her a call and ask - most of the time they'll give you an answer to that sort of thing over the phone.

place non-toxic magic marker on your reed .
play your instrument
look in the mirror....see where the ink stains the skin'
pierce below that marker

pierce below that marker Pierce below enough that you compensated for the piercing sticking out. I don't know personally, but I bet it'd be uncomfortable to have the clarinet reed hitting the piercing. Maybe not painful, but annoying.
As for when to play? I would wait until it's fully healed. The piercing place should give you a timeframe for that. Then you KNOW you're in the clear. And it's worth the wait than to risk causing something going wrong or more discomfort than needed.

A Google Image Search http://images.google.com/ on labret piercing shows that it goes through the lower lip, so that the ornament is both outside and inside your mouth.
I think it would be difficult to play clarinet with the ornament in place, and with it removed, the hole through your lip would produce an air leak.
Unless you let the piercing heal completely closed, so you can't wear the ornament, you will probably have difficulty playing clarinet.
It's your choice.
Best regards.

I was considering getting one myself but the doctor said Medicare wouldn't cover it.

Though I don't personally have a labret, a friend of mine does. Bear in mind it takes a great while for the piercing to completely heal. Facial piercings take especially long. If I remember correctly, you can't even remove the stud for at least 6 months to a year. You can probably start playing without too much irritation after a few days. I would advise rinsing your piercing after every playing session. Reeds are not known for being germ-free. As to the air leak topic, it's a non-issue, in my opinion. I have my nose pierced (twice, no less) and that's never even occurred. For air to escape the flesh would have to stretch quite a bit. You'd yelp before it got to that point. Think carefully about the piercing. Much as I love mine, I've gotten quite a bit of flack from it.
