See a doctor before you play anymore. Pain is the body's way of telling you something's wrong, and you don't want the risk of permanent damage by ignoring the signals.

Gotta agree with Mark. See the doc first.
Don't be surprised if you need a neckstrap for the clarinet. This takes a bunch of the stress off your thumb. There is no shame in using a neckstrap for a clarinet. I don't see why folks don't use them more on clarinets. It makes sense and it makes life much easier. See if your horn can be fitted with a clip-on neckstrap. Many of the newer student and intermediate grade horns come with clip loops and heavier grade metal supports, just for neckstraps. Check this option out when you have a chance. But first, see the doctor about the pain problem to make sure you aren't doing real damage to your thumb or your hand.

I recently hurt my right ring finger... I don't even know what I did to it, but I went to the Dr. and he said to wrap it and soak it in hot water... that didn't work and I now have a date to see the physical therapist on Friday because it is tendonitis in the joint. It feels fine when I don't play, but when I do it aggravates it. I don't think my clarient teacher is going to take so kindly to me cutting down on practice time, but I was told that I either could cut back now, or not play at all when I keep agravating it.
Don't play for a while. You may be out for a week right now, but if you don't take the time off now, you could be out longer because you damage it more.
Good luck and wish me the same!!!
Jo

Assuming we are talking about your right thumb, take Mark's and Paul's advice. In addition to the neckstrap, get a thumbsaver. It is simply a cushion that fits around the thumbrest and can be found at your local music store.

A couple of weeks ago I went overboard on the practicing and strained the muscle between between my thumb and forefinger. I put ice on it and took ibproufen for a few days and it really seemed to help.
Lots of Luck,
Cathy
