The "Cambridge Companion to the Clarinet" [p.158] describes alternate fingerings for C#''' to E''' using an open (or slightly "shaded" thumb hole *instead* of the 1st finger vent. These fingerings are based on the german system clarinet but "can be used to great effect in high, soft passages...".

BTW - the same book also gives fingerings (and technical notes) for a range up to g'''''. That's a full 5th higher than I ever thought was possible, giving the clarinet a total range of 4 octaves plus a min 3rd!
