It's a bis key like on a saxophone, so you can play b flat using only L first finger.

For what it is worth, I purchased one and installed it on my jazz clarinet. After the first gig, I took it off because I could not stop inadvertantly hitting and producing unwanted Bb's and Eb's. Perhaps my fingers are too fat, or I didn't give it enough time, but it just did not seem to be right for me. It is an ingenious little device that is easily installed with an "allen" wrench (no music store tech needed) which is supplied with the key. However, it is now collecting dust in my "stuff" drawer. So for me, the Runyon bis key (although well designed and easy to install) draws a negative review.

BTW, I've seen biskey attachments that are permanently soddered onto the Bb/Eb pad cup that seem to function better. I believe one was done by Charles Bay (I think he used a dime for the one on his bass clarinet). For me, however (and in my old age), the traditional clarinet Bb/Eb fingerings work just as well--I don't even use the bis Bb on the saxophone.

Morrigan,

Like it says, you can play Bb/Eb by sliding the first finger down on the BIS key and you get a Bb. Great when you would have C D Eb D C or a similar finerging. Not for use as a chromatic fingering though.
