 Bach/Hasty Transcription

Does anyone know where one can get the sheet music to Stanley Hasty's transcription of the Bach Chromatic Fantasy and/or the cd with Tom Martin performing said transcription?

I think Luyben Music has it. At least they were selling it at the ICA ClarFest last summer in Atlanta.

Earlier this year I got a copy from Albert Alphin's shop in Needham, MA - it included the CD he might have some more.
