Here's the reply I received to my email to the Bohuslav Martinu Foundation:
1. They do have a copy of the original manuscript which is deposited in the Bibliotheque Nationale de France in Paris. The version with two cadenzas has never been published.
2. The critical edition of the Oboe concerto will be published in 2009 as a part of a complete critical edition prepared by the Bohuslav Martinu Foundation & Institute.
Well, that's exciting!
