OK, Thomas, you've got me in the world of GOOGLE now...but it's amazing what one can find out!
The following link, in which Diana Doherty discusses the gaps between the printed edition and the original manuscript (which was a mess but is the only place where you can find the two cadenzas) and confirms that "everybody's got their own version", is very informative:

Also very interesting is the following website dedicated to ALL that is Martinu (a Czech site apparently maintained by his daughter (?)):

which reveals that the original manuscript is in Pamatnik Bohuslav Martinu, which upon further searching, appears to be the municipal museum in Policka, Martinu's Czech hometown...

I assume if you contact the folks at the Martinu website, they may be able to steer you to a source for a copy of the original manuscript...they're obviously out there somewhere! Btw, I highly recommend the Nancy Ambrose King recording..it's even better than the Klein and I am a very big Klein fan...I guess the bottomline is that it's up to the performer to not only interpret what's on the page but to decide what (s)he wants there in the first place! How 20th century is that?
