it turns out the first edition had numerous flaws which have never been corrected (including spelling the dedicatees name wrong (!)), possibly because martinu died a year before publication. check out the following:

i've gone back and edited my copy and listened to alex klein and nancy ambrose king's recordings of it, and interestingly what i ended up doing was taking some of the changes and leaving some of them...as i recall, alex klein pretty much played what was on the page and king made many of the changes, including restoring the two cadenzas..actually, i played this concerto back in '75 for the first time and already then my teacher made many of the changes (still marked in my copy) because they were obviously needed just going by ear, as the above article suggests everybody did. also check out this:
