Could there be something like a half-hole left index finger fingering for the upper octave? I forgot when I tinkered with a suona (Chinese shawm) whether it had a thumb hole or whether I could even get into the 2nd octave either. I think it does have a 2-octave range. A baroque oboe has no thumb hole and is played by overblowing, but of course the lips are on the reed. How large is the opening on your reed? Can you close it a little? If your bottom octave sounds very trumpety, then your reed is probably too open and therefore you won't be able to get the 2nd octave. I'm just guessing.
