A Google search on Ars Instrumentalis 75 (Sikorski's file number) turned up a number of sources, including http://www.tfront.com/p-376530-introduction-and-variations-on-a-swedish-folk-song-op-12-for-clarinet-and-piano.aspx.

It's listed on this site at http://test.woodwind.org/Databases/Composers/Names/002480.html as published by Amadeus as well as Sikorski. SheetmusicPlus lists it http://www.sheetmusicplus.com/title/introduction-et-air-suedois-varie-sheet-music/2182826.

Amadeus lists it as in print <http://www.amadeusmusic.ch/itemcard.php?itemno=BP%202512&lid=0&qsa=|||203||0|>.
