 Who makes a quality adjustable thumbrest? Not those crappy ones that come on Chinese C-O-S's but a good rugged one, preferrably with a three screw mount.

 Ton Kooiman.  Two screw mount for the etude, but solid and very adjustable.  I got one recently, and it's great! 
 
Retired, playing more sax than clarinet, but still playing clarinet and still loving it!

 Kooiman is great.  I have a Bay adjustable that works well ... but not sure if they are available anymore. 
 
Most of the issues I've had with stock adjustable thumb rests are not comfort ... usually that the thumb rest is too low and cramps the RH thumb.  I don't know why manufacturers can't move the thing up about 12mm ...?

 You could use up to six screws to mount the Kooiman Etude if you really wanted to! 
 
Just don't use the screws they supply though.
