Dear Charlie,
One of the biggest places in the States (and best prices) is Frederic Weiner Music in New York. It's actually in Long Island but if you can manage the trip, they have just about everything. They charge for each mouthpiece you try though. Must be how they make ends meet.
Frederic H. Weiner Music
168 Jericho Turnpike
Mineola NY, 11501
(516) 747-5004
(800) 622-CORK
The store front is actually "POPPA'S MUSIC" and you'll miss it if you blink.
Call Mark and arrange time to spend time if you can.

If your in the Chicago area Walter Grabner and Greg Smith would definately be worth checking out. Walter makes top notch Mouthtpieces and Barrels, as well as setting up and selling Buffet's. His stuff is really top notch. His site is www.clarinetxpress.com. I have heard great things about Greg smith's stuff, but I do not have any personal experience with him. His site is http://www.gregory-smith.com/. Also you would not be far from the Big box store of the music world, WWBW in south bend Indiana, they do have almost everything there, but impersonal compared to a real custom guy, like the previously mentioned. Hope this helps,
Ben
