 screw of metal rod on upper joint of B12 and E13

Looks like just overnight the screw of metal rod(s) on the upper joint of E13 that I use twice a day for practicing has popped up (1/8" enough of the second part of the rod became a bit of loose). I noticed this when I assembled and started my practice this morning, high C (above staff line), E & F (1st staff line & space)went way out of tune. It was all tuned o.k. yesterday afternoon's practice. I assemble and disassemble my clarinet after each practice.

I wonder what could have caused this screw pushed up all of sudden (it doesn't seem to be gradual things since I practice everyday, keep it in the case when I don't use it. I use tuner all the time when I practice). I screwed back and made sure the 2nd rod spring underneath aligned so that bridge key would work properly (initially I let it loose as it was and noticed the bridge key didn't work). Come to think of this incident, I remember I had an exact same problem with my B12 years ago (at that time, I just screwed it back and everything went o.k. since then). Summing it up:

1) what could be the possible reason/cause for the screw malfunction suddenly as I stated above?
2) Is this common known problem with Buffet clarinets?

If you mean the rod screw for the LH index finger ring key (LH1 ring key) then this screw is never fitted correctly, so it often backs out as it's never possible to get it nice and tight (but not over torqued) to keep it locked in place like any other rod screw.

Basically they haven't bothered to sufficiently countersink the end of the LH2 ring key rod which will result in that ring key binding up solid when the rod screw is tightened - it's left loose simply to allow the LH2 ring key to function.

I find I have to fit this rod screw on every Buffet I work on, along with loads of other things they can't even be arsed to do properly at the factory.
