I really like the Kooiman thumb rest I have used it for years. It can be adjusted so the thumb is above the index finger or below. It looks to me to be more than an inch of adjustment range. The screws holes match the holes in my Buffet so I did not need a tech to change it out. Just unscrew the three screws for the metal thumb rest and use two screws for the Kooiman.
But the best feature is that it moves the the weight of the clarinet from the outer thumb joint to closer to the hand and takes a lot of stress off of the thumb. The rest also gently curves around the thumb with a large surface area instead of just a small piece of metal sticking out on the thumb joint.
