C on alto sax is Eb concert.
C# on alto sax is F concert.
D on alto sax is F concert.
Eb on alto sax is F# / Gb concert.

Gnomon is right - when an instrument is a transposing instrument, the key that it is in (in this case, E-flat alto) tells you what note concert is sounded when a 'C' is played on the instrument. To clear things up a bit - when the alto sax (in E-flat) plays a written 'C', the sound that comes out is an E-flat concert. The same goes for key signatures. When the alto sax plays in the key of 'C major', the sound that comes out is in E-flat major concert. Thus, when the alto sax plays in the key of E-flat major, the sound that comes out is F-sharp/G-flat major concert.
