A or Bb, or does it matter? Was Martino after a particular sonority? How about switching between movements?

I've always played it (and heard it) on the B-flat instrument.

I would say Bb is the standard. I would love to know Martino's thoughts, and I think there could be some interesting tonal possibilities of switching, or at the very least the leaps into the altissimo may be more mellow on A clarinet (maybe a good or bad difference).

Play it on the E flat clarinet, but with all the tempi dialled up so it sounds like you're at 45... The audience will love this. Also, on the bass clarinet play it at half speed - this is great if you haven't done enough practice, or need to fill in extra time on a program.
