 What to put on a music resume

I'm applying to a summer music program that requires a music resume. I've never written one before and have no idea what to put (or not put) on it. I assume that the groups I've played in and my solo repertoire should be on there, but what about orchestral pieces I've played? I'm applying as a clarinetist, but I am also a decent violinist- should I list my violin experience?

I'm feeling awfully conformist because I'm playing the Mozart Concerto and the Weber Concertino on my tape. Oh, well...my Stravinsky and Copland are very rusty.

Keep it as concise as possible.

List those things you state in your post; groups, solo repertoire, your experience chronologically, with perhaps a short introductory paragraph about why you're applying for the summer music program.
