It's probably a Wurlitzer RB, with the throat Bb vent mechanism running off the throat G# key.
It's a variant of the usual design, a different means to the same end:
The mechanism allows for a smaller register vent, giving better-tuned 12ths (mostly), with a second hole opening for a resonant pinch Bb.
A friend has a set from the mid-80s with identical keywork.

Hi Beppe,
According to an interview in Dutch published on his website (under button 'Recensies', link: In gesprek met Henk de Graaf deel 3) Henk plays Wurlitzers. Reform Böhm Wurlitzers have been the standard for professionals in The Netherlands since several decennia.
Translation of the relevant part:
Question: You have always stayed with Wurlitzer, but on which brand did you start?
Answer: On music school and also in conservatory I played a Boosey & Hawkes Symphony XX. I found that a nice instrument. In 1971 I bought an Herbert Wurlitzer set and ten years later a new set of Wurlitzers. I am still playing on this last set.
