I have a thumb rest by Yamaha, which can be adjusted. It does not correct the thumb position, but lets me hold the clarinet with a much straighter thumb.
The reason I had to do this was a constant pain in the thumb. That pain immediately went away almost completely with the new thumb position. I do not like the idea of developping a permanent demage.
Concerning the correct position of the fingers, I was told that the thumb should face the middle finger. Now my thumb faces the first finger rather. I do not find that this hinders my playing. But I sure know that it is much better for the sanity of the articulations in my thumb.
