I've seen the question asked a few places regarding the 4th octave of the flute. The Cubans have moved to the silver boehm from the French simple system and continue to play very high - even up to F. They rarely play from 4th line D and below. Musicians like Jose Fijardo, Richard Euges and Nestor Torres (to name only a few) are like english soccer heros. Listen to Orchestra Aragon or more recently Isreal "Cachao" Lopez recordings "Master Sessions I and II.
I've got some fingerings up to 4th octave F and they are:
C Lft Hnd 1234 Rht Hnd 1 4(2)<-- 2 means low C
C# " 2 4 " 1 4(2)
D " T 3 " 12 4(1)4(2)
D# " 1T234 " 1 Tr.1 34
E " 12 4 " Tr.2 34
F " 2 " 2Tr.2
Hope these are of use. Paquito D'Rivera told me in an email to "learn the fingerings and BLOW!"
So I'll just pass this on with the hope that someone has found an easier set and may want to share them.

The fingerings on this site (sneezy.org/wfg) go up to either an extremely high g, or an insanely high g#. (By my simple terms, i mean 4th octave). Check it out.
The fourth-octave f is included in many advanced fingering charts, such as that of the Rubank series advanced volumes 1 and II.
