Sorry no idea on the patch, but the ideas he has for the Rossini are fabulous!
His ideas for pharsing and how the tune should always be present in the performance are spot on.
Fantastic video for anyone learning this piece.

One of the patches Carbonare uses has been previously identified here as a product made by Yamaha but apparently not widely distributed (for reasons unknown) in the U.S., though it has occasionally been seen offered for sale on the famous auction site.
Thomann Music in Germany carries Yamaha patches that look similar:

Interesting that you mention the Yamaha patches. I have been using the 0.5mm Yamaha patches, and I like them quite a bit. For me they present a nice intermediate between the thin 0.3 and the thicker 0.8 patches. I got mine from amazon.

Play double-lip embouchure and you don't need a patch. The world's best mouthpiece patch is the upper-lip. ha ha
