I have played the clarinet for 35 years BUT I haven't played in the last 8 years. What should I do to get myself back in practice? In the last 8 years I was focusing on the piano and the guitar.

I saw an Artie Shaw quote once where he said that it would take him one month to get back into shape to play publicly. So, that right there: accept that it will take a month of daily practice to get sounding halfway decent again. Be patient with yourself.
After that, it's all the same stuff you remember, probably. What worked for me was a routine of long tone warmups in fifths/fourths then octaves, then going through Kroepsch again, then working through a new etude book to keep myself interested, then actual music in order to practice memorization and, you know, music. The big thing is not to overdo it- your chops are going to be soft as hell and you're can't jump from nothing back to 2+ hours.

I'm with Bob - first and foremost you need a clarinet. Have your friendly repair person go over it just to make sure it's you and not the instrument.
Most important: it's supposed to be fun.

I'd start with the Hite Melodious and Progressive Studies, Vol. 1. They are relatively short and progress through all the keys. Start with the first couple studies. As soon as you can play them, move on to the next. You may actually be able to sightread some of the early ones and, as you progress, you may notice a snowball effect. When you get through the two volumes, you should be in pretty good shape.
