I can relate to being centered on the table, but for height, should the reed on a Bb clarinet be flush? Just a smidge below flush? A 1/16" below? My reeds seem to sound best at different positions, and I was wondering what seems to work best for the majority of you folks.
Any links to images would be appreciated much.

Depends on the reed - balance, and characteristics.
I start perfectly even (as I can get it..), and go from there. Weak reed slightly above, hard reed I work on, so that's not a situation, but if it were, a hard reed slightly below. There is the off center option also available.

Ideally, straight on the table and near flush with the tip ( I generally put it very slightly below the tip).
Realistically, I move it slghtly up or down if it's not behaving.

Depends on the player, reed and mouthpiece. I like to put it perfectly even with the tip when I look at it straight on equal to my eyes. If you hold the mouthpiece higher or lower than your straight eye level you won't get a true reading. ESP http://eddiesclarinet.com

Place the reed on and test. If you like the "feel" then leave it. If it doesn't feel right then move the reed and test again. As a test you might try this in a dark room and see where the reed works best. Even if you do determine that reeds for the most part work best in a certain position then I would still say the important thing is how it plays and not whether it is in your predetermined visual position. With this in mind don't spend much time with the initial placement. Place the reed on.....test....reposition ....test ...then leave it there or reposition.

Just a note - some tips are canted downward so that the tip rail actually lies LOW to the observer from the FRONT. Check from the side to be certain.

Test the reed by twisting the mouthpiece so that only one edge of the reed can vibrate. Play sfz on, say open G. Try the other side. If the reed is unbalanced, you'll hear it. Move it toward the side with the poorest response (or take it off and balance it).
If the reed is a bit soft, move it past the tip of the mouthpiece.
If the reed is a bit hard, move it down below the tip (but not below the tip rail).
