 Clarinet Glissando/smear

Can anybody tell me how a glissando (or smear) as in the famous opening to Gershwin's "Rhapsody in blue" is performed? I understand it involves sliding the fingers off the keys and holes slowly, but I have several queries:
1. Should you slide your fingers towards or away from the clarinet?
2. Do you incorporate any chromatic keys along the way?
3. What do you do when you reach the throat notes?
4. Do you use any lip bend?
5. Do you slide off you fingers one by one or all together?

I would be very grateful if anyone could answer this little problem for me.
