The movie is "Respiro."
It is an Italian movie, and the action takes place on the island of Lampedusa, 205 miles off the coast of Sicily.
Where there is underwater swimming there is always a bass clarinet solo. It's some nice stuff, and you have to keep listening while the credits roll at the end to get all of it.

Ah yes, the old "low clarinet for underwater swimming scenes" trick. And if a shark is nearby, listen for the contrabass clarinet cue.............

Okay, so maybe it's a bit of a cliche. Still, the underwater scenes are among the most delightful parts of the movie, and the bass clarinet adds to the effect.
The movie begins with some rather violent interaction between two rival groups of boys, which was a bit unpleasant to watch. Throughout the movie there is a lot of contrast between harshness and beauty on this poor but beautiful island.
The underwater scenes are particularly symbolic and dreamy, which makes the music even more delicious.
