How about this: Use a metal mouthpiece cap as a wrench?

The cap could have an L-shaped slot into which a short post (on the
mouthpiece) fits and locks with 1/8-turn, and then you can do anything
you wish to the outside of the cap in order to make it easy to grip and
rotate without disturbing the reed.
