I got braces in fifth grade, just after I'd started playing the
clarinet. Two years and one teacher later, I learned that I'd been
playing with a very strange embouchre. Aside from this, which was
quickly corrected once I knew what I was doing wrong, I had no
problems at all, aside from a little (normal) pain. As your son is an
experienced player, he probably wouldn't have this kind of problem.
My braces only hurt when I first got them on, and when I had them
adjusted once a month. My little sister, who is a beginning flute
player, has a relatively new type of braces that give her very little
pain, ever, even when she has them adjusted.

As to the surgery thing, my cousin, a flute player, had the same
problem. Her mouth was wired shut for a few weeks afterwards, so she
couldn't play, but she was back to normal within a couple months after
she could open her mouth again. I would think the circumstances would
be much the same for the clarinet, but I don't know.
