Dear Janice,

I want to add one small thing to the many good suggestions here (Kooiman
thumb rest, resting on knee are especially valuable.)

It's not clear from your post whether or not you are currently resting the
clarinet on the knuckle of your thumb (a position which contorts the hand)
or in front of the knuckle towards the nail (which allows your hand to take
a more natural position.) If you currently hold the weight of the clarinet
over the knuckle, you might try moving it away (in addition, perhaps, to
resting the bell on your knee) This is a more ergonomic and less painful
position.

As someone else suggested, with a Feldenkreis or Alexander technique teacher
can also help you relax/reposition the shoulders and arms as well to
alleviate physical stress.
