To Expand: Ton Kooiman, a Dutchman, has invented more than one style of
clarinet thumb rest for problems like your thumb. I've had one for a long
time and it was expensive, but a lot more complicated than the one written
about here. I know both models work well with messed-up thumbs. You can do
a search of Ton Kooiman to see what other styles are available -- he's got
his own web site, or at least he did a couple of years ago.

You can try the Kooiman Etude thumbrest. It take the pressure off the end
joint of the thumb by moving the thumbrest in toward the hand. I used to
use a neckstrap; I no longer need to with this device. They are only about
$30. You can install it yourself fairly easily, or have a technician do
it.
