My worst doubling quick switch is clarinet to/from flute/piccolo (pretty much anything to/from flute/piccolo, especially piccolo). In college, I played the Reed I book in the musical "Big". It was mostly flute/piccolo with some clarinet and alto sax (I didn't play the sax parts; I didn't have one at the time and we got an alto/soprano sax player from the nearby high school).

Last year in orchestra, we played Bizet's L'Arlesienne #2. I played alto sax for the first 3 movements, then A clarinet (no sax part, so I doubled Cl-II) for the 4th movement. That wasn't bad since I had ample time to switch. For our holiday concerts this year, I played Bb, Eb, and bass clarinets. The switch from Eb to bass between pieces was the toughest. Maybe I'm just used to it, but I don't have a problem going back and forth from Bb/A to Eb in the midst of a piece.
