Hi Charlie,
Try setting up both instruments in your practice room and work on
going back and forth. Make all necessary changes in embouchure, air stream as
quickly as possible. As you pick up the other instrument, visualize in your
mind proper playing aspects. And yes, as someone else mentioned, work on
each separately to make sure your playing is strong on both.
