Perhaps it would be good to say that the ranking is "absolute", not
"relative". If none among the finalists reaches the highest required
minimum level, the first prize is not assigned.

Setting the (absolute) scale is the work of the jury and makes the
competitions different. A prestigious competition has a really high
"normalization" for the absolute scale, while somewhat lower levels are
required for more common competitions.

The organizers who wish to set up a tradition of high-prestigious
competitions need to choose a good jury, make a lot of world-wide
advertisement, and offer appealing prizes. It can well happen that a
competition is a failure because the musicians who would be at the
"correct" level did not notice it.

In addition, as you said, the success of the winners in their
professional career is another ingredient which contributes to
establishing a good name for the competition.
