The guy playing the clarinet onscreen in the Godfather was most
likely an EXTRA, with no relation whatsoever to the person playing
clarinet on the soundtrack. Extras casting agencies often stipulate
playing ability (and ability to furnish one's own instruments) when
casting onscreen bands/orchestras, and there are plenty of SAG-card-
carrying musicians who do that kind of work. There are exceptions (as
in "The Aviator," where Vince Giordano's Nighthawks furnished
soundtrack music AND were filmed onscreen "playing" it). More often,
though, it's actors synching their "playing" to a recorded track, as
in the recent "The Good Shepherd," where Matt Damon's character sings
live to a prerecorded Gilbert & Sullivan track; there, the filmed
orchestra were actual musicians with their instruments, reading the
actual parts (which were provided), but faking the "playing" part.
