One issue that didn't come to mind when I made my original post was that
I have a hiatial hernia. My understanding is that this is a perforation
of the diaphragm by the stomach muscle. Feel free to correct the
anatomy, if I've got it wrong. I'm also wondering if this will have an
impact on how I breathe. So far I do not feel any pain.
