This has been solved simply by Wurlitzer on their Schmidt Reform Boehm
clarinets, by adding an extra vent key on the bell, operated by the RH
thumb, which you can use on low E and F. On these clarinets the tuning
adjustment is tiny anyway so I rarely use it, but it is there.

Someone like you could add this to a standard Boehm system.
