The following website gives a summary of a
dissertation written by Christopher Allan Gibson in
1995 regarding the soft palate air leak in
clarinetists. Perhaps it could be of some use to you.

I myself have this same problem on occasion and
believe that it may be caused by fatigue because it
USUALLY occurs after excessive practicing or after I
have been sick. However, the only solution that I have
found is to stop practicing immediately when it begins
to occur because the longer I continue to play the
worse it gets and it often becomes quite painful.
