The examples I can remember offhand are:

Entering while the piece is being performed.
Leaving their cell phone ringers on.
Talking on their cell phones during the performance .
Making lots of crinkling sounds with wrappers.
Excessively coughing.
Clapping between movements

Other amusing calamities happen throughout the piece, although not all are
necessarily indicative of inappropriate concert etiquette. All the above
items and more I cannot recall are scored in the music. Worthy of P.D.Q.
Bach, but I'm certainly biased.
