Keep your right hand thumb pressing the clarinet up and into the top teeth
and embouchure. Try to keep your clarinet set into the embouchure without
movement.

Be sure that you are placing the reed correctly on the mouthpiece table.

Try a different mouthpiece (of quality) and see if the problem continues.
