These questions are next to impossible to answer specifically in email. All
of those factors could be the problem, or none of them.

The first thing you should do is try a professional instrument known to be
in good repair status. Perhaps your teacher will let you try his instrument
(with your mouthpiece). Perhaps your teacher would be willing to play on
your instrument with his or her mouthpiece. This will give you a lot of
information.

Or, there are mail order companies which will ship you instruments for
trial. This doesn't mean you have to buy them. If you give the list an idea
of where you live, someone may be able to suggest a store which stocks
professional clarinets for you to try (sadly becoming less of an option).
Whatever you do, it never hurts to try instruments out.
