I have a copy of volume 3, bought many years ago here in the UK in a fit of
hubris (I still can't really play any of them!)

The publisher was Boosey and Hawkes, and it bears the number "B. & H. 9078".
A search of their web site found it at
<http://www.boosey.com/pages/shop/product_detail.asp?id=618001&a=>
where it is shown as "Custom Print" - (" hand-made for you (normally
non-returnable), and take about 14 days to be despatched."). They take
orders worldwide by credit card.
