The underlying issue is that health insurance is not mandatory for every
person. Thus the cost of healthcare is not shared equally by everyone.
A large number of individuals thereby get screwed.

Insurors can refuse to offer coverage, albeit sometimes by more complex
method than simply saying "no", and individuals who consider themselves
'too healthy' to need insurance can refuse to buy into the system.

There are many, many other aspects of health insurance to discuss, such
as whether individuals should pay the premiums directly or should the
government pay for health care via taxation? what about non-citizens who
receive health care but don't pay for it? and so forth.

But at the very foundation, the problem is that every individual and
insuror and employer (and so forth) is allowed to opt out of both pubic
and private health care systems. So those who 'stay in' pay too much,
and those who 'want in' but cannot buy their way in are defenseless
against inflated charges from providers who make up for the free care
that they provide and/or for reduced compensation that public opinion
forces on them, and so forth.
