I had a very good student with extreme problems (inherited from an
accident). I persuaded her to use a Kooiman thumbrest (Maestro). It worked
extremely well for her. It does take a bit of setting up, and it does mean
re-working at the technique of using the side trill keys. However it does
work in its stated aim of taking the pressure off the thumb.

Once set up, she has never looked back and has gone from not being able to
play for more than about 5 minutes at 15 to graduating with an honours
degree in Performance at 21. I think that is testament to the worth of the
Kooiman thumbrest.
